A Weight, blood pressure, falls, mood and appetite are watched over time, not just at appointments.

Active provider communication

A Your nurse talks with doctors, specialists, pharmacies and facilities, and follows up on what was said.

Family communication and updates

With your permission, family members hear what changed and what happens next.

Appointment preparation and coordination

Questions prepared, records gathered, and follow-up organized. Your nurse can join appointments when it helps.

Care plan development and adjustment

A Your plan, medication list and goals are updated after every appointment, hospital visit and change.

Priority scheduling

You are first in line when something changes and you need time sooner.
