Concierge is our highest level of support, designed for complex situations and for families who want a registered nurse deeply involved, whether you live across town or across the country.

HIGHEST LEVEL OF SUPPORT

$1,350/ month


8 hours      of nurse time included each month

3 months   minimum after your assessment

$200/hr     for any additional time, billed in 15-minute increments

Everything in Comprehensive, with more of your nurse's time, closer coordination and a faster response, all built around your Personalized Care Plan.

Biweekly check-ins

Regular scheduled contact with your nurse, so changes are caught early and questions don't wait.

Monthly Family meeting

Everyone together, in person or by video, to review progress and agree on what comes next.

Hospital and rehab transition support

Help understanding discharge plans, medication changes and what needs to be in place at home. 

Facility coordination

Communication with assisted living, rehab and skilled nursing teams about care and concerns.

Facility research and tours

Options narrowed to what fits, with your nurse attending tours and asking the clinical questions.

Increased provider communication

More frequent contact with doctors, specialists and pharmacies, with follow-up on every conversation. 

Increased family coordination

Updates reach the people who need them, including siblings who see the situation differently.

Appointment participation

Your nurse joins visits when it helps, so important details are heard and written down.  

Ongoing care-plan oversight

Your plan and medication list stay current after every appointment, hospital visit or change.

Next-business-day response

Scheduling and care-management messages answered by the end of the next business day.

Concierge care management does not include a 24/7 nurse line or emergency response. In an emergency, call 911.
